Nomination Form – Fauquier Valor Awards

Use this form to submit nominations for the Fauquier Chamber Valor Awards. Required fields are marked with a * and at least one nominee is required per submission. Some awards are specific to be presented to full units or individually. If additional nominees are required please click the "add" button below the nominee submission, add as many as you wish. All submissions are subject to review and verification through department leadership as well as the Valor Awards Committee.

Military Veteran Nomination - One or more Veterans can be submitted for consideration.
Meritorious Unit Award -- Single event with multiple personnel recognized as a unit.
Meritorious Service Award -- One or more personnel to be recognized individually.
Life Saving Award -- One or more personnel to be recognized individually or as a unit.
Valor Award - One or more personnel to be recognized individually or as a unit.
Lifetime Achievement Award - One or more personnel to be recognized individually.

SELECTED: Military Veteran Nomination

Criteria – Each year a Military Veteran is hornored for their service to our great country. They must either work or live within Fauquier County. You can submit mulitple Veterans at once but only ONE veteran will be selected and honored at the event.

SELECTED: Meritorious Unit Award

Criteria – This award is provided to a full unit that was involved in an incident or incidents that involved extra ordinary efforts to rescue, defuse and/or create a safe and secure situation.

SELECTED: Meritorious Service Award

Criteria – This award is provided to individuals who have demonstrated extra ordinary efforts either at a specific incident or through a series of investigative endeavors.

SELECTED: Life Saving Award

Criteria – This award is provided to individuals who, through their extra ordinary efforts, were able to take actions that resulted in the saving of a life or lives.

SELECTED: Valor Award

Criteria – This is the greatest award we provide. Unfortunately, in a lot of cases it is awarded posthumously. This is awarded to an individual who faced extreme danger to save the life or lives of others.

SELECTED: Lifetime Achievement Award

Criteria – This award is provided to an individual who has demonstrated a long-time commitment to serving public safety. This can be a current or retired individual. The term “long-time” will be determined by the committee with feedback from each of the department heads. Probably 30+ years.
